Insurance

Dr. Joseph Militello is a participating provider with most major health insurance plans, including MEDICARE, HMO's, PPO's, and Workman's Compensation. We have provided a listing of our current insurance plans below. If you don't see your plan listed, please call our office at 352-683-4500 to see if we participate with your insurance.

It is the policy of The Eye Center to collect co-payments and deductibles at the time of service per the specifics of your insurance plan. Payments may be made by cash, check, MasterCard or Visa. For procedures not covered by insurance, our Billing Specialist will work with you to create a payment plan.

The Eye Center will file your primary and secondary insurances for you. We ask that you always keep our office updated with any name, address, or insurance changes so we can assist you with your claims.

The Eye Center accepts MEDICARE and all secondary insurances that cover your portion of the Medicare costs. We will file your primary MEDICARE claim and submit to your secondary insurance as well.
